What medications are used to cure Alzheimer's disease?
Four medications, tacrine (Cognex), donepezil (Aricept), rivastigmine tartrate (Exelon) and galantamine (Reminyl), are used to improve intellectual function in some people with mild to moderate Alzheimer's disease for periods up to 18 months. These drugs (called cholinesterase inhibitors) increase the brain's levels of acetylcholine, which helps to restore
communication between brain cells. Another medication, memantine (Namenda), has been shown to stabilize memory in people with moderate to severe Alzheimer's disease. It is the first in a new class of medications called NMDA receptor antagonists.
Aricept is the most widely used drug for Alzheimer's disease. Aricept works by slowing down the breakdown of acetylcholine, a chemical that helps nerve cells in the brain communicate with each other. Aricept doesn't cure Alzheimer's or keep it from getting worse, but it can help relieve some of the memory loss. Aricept is most effective when given in the earlier stages of Alzheimer's disease. Side effects are usually mild and include diarrhea, vomiting, nausea, fatigue, insomnia, and weight loss.
Tacrine (Cognex) was the first cholinergic protective drug. It needs to be taken four times a day, has only modest benefits, and has no benefits for patients who carry the ApoE4 gene. Side effects of the drug include nausea, vomiting, diarrhea, abdominal pain, skin rash, and indigestion. In addition, tacrine can damage the liver, so your doctor will need to perform tests regularly to make sure this isn't happening. Because of these side effects, this drug is infrequently prescribed. In general, newer cholinergic protective drugs that do not pose as great a risk for the liver are now used for Alzheimer's.
Donepezil has two advantages over tacrine: It has fewer side effects, and it can be given once daily rather than three times daily. Donepezil does not appear to affect liver enzymes, and therefore does not require weekly blood tests. The frequency of abdominal side effects is also lower. The monthly cost is approximately the same.
Estrogen, the female sex hormone, is widely prescribed for post-menopausal women to prevent osteoporosis. Several preliminary studies have shown that women taking estrogen have lower rates of AD, and those who develop AD have a slower progression and less severe symptoms.
Preliminary studies have also suggested a reduced risk for developing AD in older people who regularly use nonsteroidal anti-inflammatory drugs (NSAIDs), including aspirin, ibuprofen, and naproxen, although not acetaminophen.
Selegiline, a drug used in the treatment of Parkinson's disease, appears to slow the development of AD. Selegiline is thought to act as an antioxidant, preventing free radical damage. However, it also acts as a stimulant, making it difficult to determine whether the delay in onset of AD symptoms is due to protection from free radicals or to the general elevation of brain activity from the stimulant effect.
Another antioxidant, vitamin E, is also thought to delay AD onset. Antioxidants are substances that combat the effects of oxidative damage in our bodies. Oxidative damage to proteins and DNA occurs when our cells utilize oxygen to produce energy. Toxic byproducts of that process are called reactive oxygen species, a kind of free radical. Natural substances in the body (antioxidants) act to scavenge these dangerous molecules and render them less harmful, but this protection fails over time. Oxidative damage has been implicated as a possible cause or contributor to the damage in Alzheimer's disease.
